Abstract Details

Purpose:

To compare the in vitro optical quality of two new trifocal intraocular lenses (IOLs) at different degrees of decentration using an optical bench.

Setting:

University of Valencia and University of Granada, Spain.

Methods:

The AT LISA tri 839MP with trifocal diffractive design (Carl Zeiss Meditec) and the FineVision IOL (PhysIOL), which combines two distinct apodized bifocal diffractive structures resulting in three foci were evaluated. The optical quality of the both multifocal IOLs was assessed in vitro using a PMTF optical bench (Lambda-X Co) with an aberration-free model cornea. This equipment allows to measure the modulation transfer function (MTF) by imaging an edge and then, processing the image. The two trifocal IOLs were evaluated both centered and decentered (0.2 and 0.4 mm). At each position, the MTF for each focus and the through-focus MTF at least at 10 different focal points (steps of 0.5 D) were measured for 3 and 4.5 mm aperture diameters.

Results:

The through-focus MTF curves showed three peaks for both trifocal IOLs, corresponding to far, intermediate, and near foci, respectively, when the measurements were performed for 3 and 4.5 mm apertures and for both centered and decentered positions. However, for both lenses we found a worse optical quality for higher aperture diameters. We also found that with simulated IOL decentration, the optical quality of both trifocal IOLs was deteriorated, showing a higher impact for larger aperture diameters.

Conclusions:

The two trifocal IOLs offer comparable optical performance. However, small decentrations after IOL implantation could deteriorate the optical quality of these multifocal IOLs, especially when larger pupil sizes are considered, and therefore, to affect to the patient`s visual performance, particularly for activities which require low-illumination levels.
