Abstract Details

Purpose:

Current methods to align toric IOLs are either imprecise or expensive. We report our experience with a new method that combines simplicity and precision.

Setting:

Ophthalmic Unit, Dept. of Neurosciences, Hospital and University of Verona, Italy

Methods:

Toreasy is a new software for the intra-operative alignment of toric IOLs using the internal accelerometer of smart phones or pads. Pre-operatively the head was positioned horizontally by aligning the canthi of both eyes of the patient with the horizontal line superposed to the image of the patient’s face obtained with the smart phone. A snapshot of the patient’s eye in this position was taken. Anatomical markers at the limbus were chosen by the surgeon with their exact angular position given by the software. The Mendez ring was then used intra-operatively to align the IOL in the desired orientation with respect to the anatomical marker. Twenty trifocal toric IOLs were implanted using this new alignment method. Vectorial analysis of the refractive outcomes as well as visual acuity outcomes were reported.

Results:

This software solution enabled IOL placement in every case. The IOL orientation was correct and the pre-operative corneal astigmatism of 1.78±0.76 D was reduced to 0.38±0.35 D (refractive) in the postoperative. Uncorrected visual acuity at near, intermediate and far distance was above 0.2 logMar in all cases.

Conclusions:

This alignment method is efficient, costless and more precise than usual marking. This is particularly important in the case of complex optics IOL. The combination of Toreasy software with trifocal toric IOLs produced a very satisfactory outcome in this study.
