Abstract Details

Purpose:

To evaluate the long-term efficacy and outcomes of capsular bending ring (CBR) in Japanese eyes, especially in preventing long-term posterior capsule opacification (PCO) in visual axis.

Nishi Eye Hospital

Methods:

Randomly selected 60 patients with senile cataract underwent 15 years ago in one eye cataract surgery with implantation of a classic CBR. The CBR is an open band PMMA ring with a sharp edge designed to induce a sharp bending in the equatorial capsule and prevent anterior and posterior capsule from being in contact. The other control eye did not receive the ring. Clinically significant PCO measured by the rate of YAG capsulotomies as well as complications were investigated. Retroillumination photos were taken to evaluate the IOL clarity in visual axis.

Results:

The whole group of 60 patients consisted of 35women and 25 men. The mean age was 69 years at the time of cataract surgery and the mean IOL power was 21.8 D, ranging from 19.0 to 24.0 D. Each patient was contacted to check if the patient could come for checking-up. So far, 6 patients could come, whose eye with the CBR showed PCO free optics, although the control eye had already YAG treatment. Just as an example, one patient, who underwent cataract surgery at the age of 49 years underwent YAG capsulotomy in one eye without a capsular bending ring just 2 years after surgery, whereas in the other eye with the ring, even 15 year postoperatively, IOL in visual axis was totally free from PCO. The updated data will be presented.

Conclusions:

The classic CBR was previously reported to reduce clinical PCO significantly during the early postoperative period, thus probably be useful for cases of vitreoretinal surgery or pediatric cataract. The ring seemed to play its role well also during the long term period at least in keeping the visual axis clarity from PCO. Combined with Femtosecond laser treatment for precisely centered CCC, the ring might play an effective role in preventing PCO in center during the long term postoperative period. The precise morphological evaluation would be necessary as a next step and the recently demonstrated concept “open capsule” effect should be investigated carefully.
