To investigate the clinical results of pseudophakic monovision depending on postoperative spherical power of the eye for near vision

Among the patients who had bilateral phacoemulsification and monofocal intraocular lens (IOL) implantation between February 2010 and May 2013, this prospective randomly assigned study evaluated 60 patients who had implantation of different targets of monofocal IOL for each eye. For all patients, the eye with the more severe cataract had the surgery first and was corrected for distance vision targeted to 0 ~ -0.50D range. The fellow eye was operated 2-4 weeks after the first operation and corrected for near vision targeted to -1.50 ~ -2.50D range considering patients’ near task. The refractive error, binocular uncorrected distance visual acuity (UCDVA), uncorrected near visual acuity (UCNVA), best corrected visual acuity (BCVA), and stereopsis was examined before and 2 months after the bilateral cataract surgery. All patients answered questionnaires to evaluate patient satisfaction and spectacle dependence 2 months after the surgery.


Postroperative refractive errors were -0.50~-0.75D in 23 patients (group 1), -1.00~-1.25D in 17 patients (group 2), over -1.50D in 20 patients (group 3). There was no significant difference in mean postoperative UCNVA (logMAR, group 1: 0.214 ± 0.157, group 2: 0.201 ± 0.114, group 3: 0.130 ± 0.117, p=0.074). Also there was no significant difference in mean postoperative BCVA, stereopsis, satisfaction of near and distant vision and spectacle dependence.


There are no significant differences in clinical results according to postoperative spherical power in modest myopic defocus. Therefore, considering proper amount of monovision in the light of patient’s own status like age, lifestyle and job could improve patients’ satisfaction after monovision cataract surgery.
