Abstract Details
Purpose:
The posterior capsular wrinkles can sometimes be observed after cataract surgeries. However, mechanisms of the wrinkles are not well known. In this study, we try to make clear the mechanisms of posterior capsule wrinkles.
Setting:
Department of Ophthalmology, Dokkyo Medical University, Tochigi, Japan.
Methods:
Subjects comprised 66 eyes that underwent phacoemulsification. Single piece IOL (NY-60, HOYA) 36 eyes or three piece IOL (PY-60AD, HOYA) 30 eyes were randomly implanted. The mean age of patients is 72.3±7.4. Images were taken using an EAS-1000 (NIDEK) after 1 week, 1, 3, 6 months and 1 year postoperatively. And the patients were divided into a wrinkle group (the wrinkle was recognized in posterior capsule) and a no-wrinkle group. In the wrinkle group, the relationship between the position of wrinkle and the position of optic-haptic junctions were analyzed. And also the axial length were compared in two groups.
Results:
The wrinkle was observed in 1 eye of NY-60 group (2.8%), 11 eyes of PY-60AD group (36.7%). In NY-60 group, the wrinkle was appeared at 1 month postoperatively, and in PY-60AD group, wrinkle were appeared immediately after surgery and were not decreased for 1 year postoperatively. In the wrinkle group, the direction of wrinkle were placed at an angle of 55.4±3.6 degree from optic-haptic junctions regularly. And we find that the wrinkle appear at the angle of haptic contacted with a lens capsule. The axial length were 23.47±1.03 mm in the wrinkle group and 24.50±1.24 mm in the no-wrinkle group. There were statistically significance in two groups (P = 0.02).
Conclusions:
The ratio of wrinkle incidence differed depending on the shape of IOLs. The wrinkle appears in the IOL having high compressive strength. The wrinkle occurs in short axial length eyes that have small lens capsules. The wrinkle is placed to direction of haptic contacted with the lens capsule.
