Abstract Details

Purpose:

Corneal stromal dystrophies, such as Groenouw type 1 (granular) and type 2 (macular), cause progressive decrease in visual acuity and recurrent erosions (type 2). In the past, penetrating keratoplasty has been frequently adopted, but photherapeutic keratectomy (PTK) represents a valid therapeutic alternative. We present long-term follow up of Groenouw corneal dystrophy treatment with PTK.

Istituto Clinico Humanitas, Milan, Italy

Methods:

Twenty-nine eyes of 20 patients with Groenouw corneal dystrophy (type 1, 44.8%, type 2, 55.2%) severely hindering visual acuity underwent PTK, smoothing with masking fluid and 0.02 % mytomicin application. In two eyes the dystrophy had recurred after previous PK.

Results:

Preoperative visual acuity was 0.26 ± 0.24 with +1.06 ± 2.12 D sph, -1.95 ± 0.37D cyl at 110 ± 51.11 degrees. Mean pupillary K was 45.16 ± 4.38 D. Five years after surgery, visual acuity was 0.65 ± 0.12 with +1.50 ± 1.90 D sph, -1.71 ± 0.90 D cyl at 119± 38.15 degrees. Mean pupillary K was 42.72 ± 2.51 D. No patient reported recurring of painful corneal erosions.

Conclusions:

Five years after PTK, eyes with Groenouw type 1 and 2 showed marked improvement in visual acuity, stability of refraction, and no recurrent erosion. Mean pupillary K value had decreased. Furthermore, PTK can be repeated if necessary when residual corneal thickness is sufficient.
