Abstract Details

Purpose:

To evaluate the accuracy of different keratoconus detection indices of the rotating Scheimpflug camera (Oculyzer II equivalent to Pentacam HR) software version 1.17r91

Setting:

Al-Watany Eye Hospital, Cairo, Egypt

Methods:

Retrospective study including 203 eyes (111 normal eyes and 92 keratoconus eyes), imaged by Oculyzer II. Slit lamp examination, refraction, Scheimpflug images, indices and follow up data were collected. The area under the receiver operating characteristic curve (AUROC) was calculated for different keratoconus detecting indices.

Results:

The indices with highest AUROC were Posterior Elevation from Best Fit sphere (BFS) calculated for 8 mm (PE8), that calculated for 9 mm (PE9), that from best fit toric ellipsoid (HPE), Pachymetry Progression Index (PPI)-Max, Ambrosio Relational Thickness (ART)-Max, Belin-Ambrosio Enhanced Ectasia Display (BAD) overall deviation (D), BAD PPI deviation (Dp), PPI-Avg, ART-Avg, and Index of Height Decentration (IHD) (AUROC = .996, .995, .995 .990,.989, .984, .982, .982, .982, .967, respectively). None of these indices was statistically superior than others (P>.05), but they were statistically superior than all other indices, such as other BAD indices, Anterior elevation from BFS calculated for 8 and 9 mm and from BFTE, Kmax, index of surface variance (ISV), thinnest point pachymetry and its relative pachymetry (P< .05).

Conclusions:

The most useful rotating Scheimpflug imaging indices with highest AUROC to detect keratoconus are PE8, PE9, HPE, PPI-Max, ART-Max, PPI-AVg, ART-Avg, BAD deviation indices (D, Dp), and IHD
