Corneal sensation and self-reported dry eye symptoms: a comparison between inverted side cuts and standard side cuts using two femtosecond lasers for myopic LASIK

Abstract Details

Purpose:

To prospectively compare corneal sensation and patient self-reported dry eye symptoms after femtosecond LASIK using the AMO Intralase iFS 150 femtosecond laser to create a 130 degree bevel-in inverted side cut in one eye and the AMO Intralase FS 60 to create a conventional 70 degree beveled-out side cut in the fellow eye.

University based refractive surgical service

Methods:

One-hundred and twenty-two eyes of 61 consecutive patients were treated with wavefront-guided LASIK. One eye had a flap created with the AMO Intralase FS60 laser system and the fellow eye had a flap created with the AMO Intralase iFS 150 laser system. A 70-degree bevel-out side cut was used with the FS60 laser system and an inverted 130-degree bevel-in side cut was used with the iFS 150 laser system. Both lasers had a programmed flap thickness of 105 microns with a planned 9.0 mm superior hinged flap. Eyes were randomized according to ocular dominance. The mean pre-operative spherical equivalent refraction was -4.30 +/- 2.31 and -4.32 +/-2.31 in the iFS 150 group FS60 group respectively. Cochet-Bonnet aesthesiometry (CB) measured corneal sensation pre-operatively and at postoperative months 1, 3 and 6 and 12. A validated Rasch-tested, linear-scaled dry eye questionnaire was administered preoperatively and an post-operative months 1, 3, 6 and 12.

Results:

Corneal sensitive was markedly reduced at post-operative month one with no significant differences between the two groups (p=0.118). There was a greater reduction in corneal sensation in the FS 60 treated 70-degree bevel out group compared to the iFS 150 treated 130 degree inverted side cut at months 3, 6 and 12. Cochet-Bonnet aesthesiometry values were 1.84 versus 2.49 (p < 0.001) at 3 months and 4.26 versus 5.12 (p< 0.001) at 6 months and 5.83 versus 5.98 (p=0.006) in the 70-degree beveled out side cut and the 130 degree inverted side cut respectively. There were no significant differences in self-reported dry eye symptoms between the two groups.

Conclusions:

Femtoscond LASIK using the iFS 150 laser with a 130 degree inverted side cut had faster recovery of corneal sensation compared to femtosecond LASIK using the FS 60 laser with a 70 degree beveled out side cut. There were no differences in self-reported dry eye symptoms between the 2 groups.
