Abstract Details

Purpose:

To evaluate functional visual results after a mixed implantation of two different diffractive intraocular lenses ( IOLs) in the same patient.

Setting:

Private clinic , Barcelona, Spain

Methods:

A prospective study . Preoperatory corneal astigmatism was lower or equal to 1 Diopter. Eyes with pathology other than cataract were excluded. Patients received a hydrophobic acrylic diffractive lens with a different near addition in each eye . At 3 months post op uncorrected and distance correcte visual acuities were checked for distance, intermediate (70 cm) and near (40 cm). A defocus curve monocular and binocular was done . Contrast sensitivity and pupilar diameter in fotopic and mesopic conditions were mesured. Patient satisfaction and spectacle independece was evaluated with VF-14 test.

Results:

The study enrolled 37 patients .The first operated eye was implanted with a Acrysoft Restor +3 and the second eye with a Acrysoft Restor +2,5 . All surgical procedures were uneventful. At three months post op the mean values in the +3 group, +2.5 group were , respectively: +3 monocular group UDVA 0,065 , UIVA 0,382 , UNVA 0,214 , CDVA 0,0065 ,CDIVA 0,414 , CDNVA 0,2; +2,5 monocular group UDVA 0,058 , UIVA 0,391. UNVA 0,402, CDVA -0,01, CDIVA 0,37, CDNVA 0,37 ; There was no significant differences in contrast sensitivity. Defocus curve for +3 IOL showed two peacks at 0 and 2,5 Diopters and one deep at 1,5 D. . Defocus curve for +2,5 IOL showed a high peack at 0, a smooth slope to 1,5D and a lower second peack 2D. Binocular defocus curve had better results than just summatory monoculars profiles.

Conclusions:

Eyes implanted with a +2,5D IOL had a better distance visual acuity than +3 D IOL but worse visual acuity for near. There was no significant differences in intermediate visión . There was an high degree of satisfaction , less complaints for halos or intermediate visión but complaints for lost of visual acuity at near in dim conditions persisted.
