Abstract Details

Purpose:

To report the outcome of five cases of post-traumatic full thickness corneal laceration of varying sizes repaired by using fibrin glue.

Setting:

Vasan Eye Care Hospital

Methods:

Five consecutive cases of corneal perforation of 2.2 to 8.0 mm with shallow anterior chamber were included in the study. First two cases had 2.2 mm corneal wound involving the visual axis. Case-1 had intra-corneal wooden foreign body with traumatic cataract. Third and fourth cases had 6.0 mm tongue-shaped perforation, whereas fifth case had 8.0 mm perforation with iris prolapse. Fourth and fifth cases had traumatic cataract also. In third and fourth cases one 10-0 nylon was placed to secure the wound initially. In all cases anterior chamber was filled with air through separate side port. Wound areas were kept dry, followed by placement of fibrin glue (TISSEEL, Baxter Healthcare Corporation, Glendale, CA). All patients received 300 ml intravenous mannitol of 20% in the recovery room.

Results:

Postoperative period was uneventful with single suture removal on 7th day for third and fourth cases. Main outcome measurements were wound healing, astigmatism profile and visual recovery. Wound apposition was excellent with minimal scarring and early visual recovery. All patients gained BCVA of 20/20 with or without cataract surgery. Topography showed astigmatism within acceptable limit.

Conclusions:

Thus the reported technique is a simple, less time consuming as well as effective way to manage corneal laceration of varying sizes. We need a long term study with more number of cases.
