Abstract Details

Purpose:

A prospective, randomized, double masked, clinical trial was designed to evaluate visual acuity, contrast sensitivity, visual quality, quality of life and spectacle independence in 100 patients symmetrically and randomly implanted with five different multifocal IOL designs (AcrySof ReSTOR SV25T0, AT LISA 809M, AT LISA TRI 839MP, Tecnis ZKB00 and Tecnis ZLB00), three months after cataract intervention.

Setting:

Ophthalmology Department of Santa Creu and Sant Pau Hospital, Barcelona, Spain.

Methods:

Patients were symmetrically and randomly implanted with AcrySof ReSTOR SV25T0 (n=20), AT LISA 809M (n=20), AT LISA TRI 839MP (n=20), Tecnis ZKB00 (n=20) and Tecnis ZLB00 (n=20). Three months after lens implantation, visual acuity was evaluated by assessing monocular corrected distance visual acuity (CDVA), monocular distance corrected near visual acuity at 40 cm (DCNVA) and monocular distance corrected intermediate visual acuity at 60 cm (DCIVA). Contrast sensitivity evaluation under photopic, mesopic and mesopic with glare conditions was performed with the CSV-1000 contrast sensitivity test (Vector Vision). Finally, subjective quality of vision (frequency of undesirable photic phenomena such as shadows around letters or ghosting, halos and glare, as well as night driving difficulties), quality of life (VF-14 questionnaire) and spectacle independence at distance, intermediate and near vision were evaluated.

Results:

CDVA was better with the Tecnis ZLB00 lens (0.062 logMAR) than with the AT LISA 809M lens (0.15 logMAR) (p < 0.05). For intermediate vision, the Tecnis ZKB00 lens offered the best visual acuity (0.76 decimal) when compared with all the other lens types, with the AT LISA 809M lens displaying the poorest performance at this distance (0.57 decimal) (p < 0.05). Finally, the Tecnis ZLB00 lens provided the best DCNVA (0.78 decimal). The low addition of the AcrySof ReSTOR SV25T0 was found to decrease spectacle independence for near vision. Contrast sensitivity was similarly compromised in all multifocal IOL models, particularly for mesopic and mesopic with glare conditions. The average score of the VF-14 test was 96% for all IOL models.

Conclusions:

The present results, which reflect IOL characteristics in optics, profile and add power, may contribute to help surgeons decide on the type of IOL most suitable for each individual patient, particularly those with high visual demands at near and intermediate distances.
