Two year follow-up of PCO development and glistenings in 2 hydrophobic acrylic 1-piece IOLs: an intra-individual study

Abstract Details

Purpose:

To compare development of PCO and glistenings for two hydrophobic acrylic intraocular lenses two years after implantation

Setting:

Department of Ophthalmology, Linköping University Hospital, Sweden

Methods:

To this intraindividual, prospective randomised study participants with bilateral cataract planned for immediate sequential bilateral cataract surgery (ISBCS) were invited. Exclusion criteria were other visually disturbing conditions, previous intraocular surgery, or any complication damaging lens capsule or zonulae. 50 patients participated. The first operated eye was randomised to receive either an AcrySof IQ intraocular lens or a Tecnis One-Piece intraocular lens. Visual acuity and intraocular pressure measurements were performed as well as a slitlamp examination and photography 1 week and 2 years after surgery. PCO was measured using POCOman software. Glistenings in the IOL optic were measured semiquantitatively with a 4 graded scale.

Results:

At 2-year follow-up 44 patients returned for examination. 6 patients died before 2-yer follow-up. 1 eye (with a Tecnis IOL) has been treated with Nd-YAG-laser capsulotomy. Mean POCOman area score was 11.0% for Tecnis eyes and 12.5% for AcrySof eyes, and mean severity score 0.15 for both groups. Glistenings appeared in 2 Tecnis IOLs (Grade 1), in the other 42 Tecnis IOLs grade 0 glistenings were detected. 8 AcrySof IOLs display grade 0 glistenings, 3 AcrySof IOLs each were graded 1 and 2. 7 AcrySof IOLs showed grade 3 glistenings and 23 AcrySof had glistenings grade 4. Glistenings were not linked to any visual complaints.

Conclusions:

Two years after cataract surgery, an intraindividual comparison between AcrySof IQ and Tecnis 1-piece IOLs reveal equal results regarding PCO development. Glistenings are readily observed in most AcrySof IOLs whereas only two of the 44 examined Tecnis IOLs displayed sparse glistenings. In the study population both IOLs provided equally good visual results and satisfying visual quality.
