Abstract Details

Purpose:

Assessment of the visual acuity and refractive correction achieved by amblyopic eyes undergoing collamer phakic posterior chamber intraocular lens (ICL) implantation versus corneal refractive laser surgery

Setting:

Clinica Baviera. Instituto Oftalmologico Europeo. Spain

Methods:

This retrospective comparative study enrolled moderately myopic eyes with non-organic amblyopia undergoing refractive surgery (laser vs phakic lens). For the purpose of this study amblyopia was defined as a best spectacle-corrected visual acuity (BSCVA) below or equal to 0.7 (Snellen), and moderate myopia as a sphere equivalent (SE) within -5 and -10 diopters (D). Eyes with preoperative refractive cylinder greater than 2 D, those requiring bioptics or which suffered any intra or postoperative complications, or those for which emmetropia was not the refractive aim, were excluded from this study. The minimum follow up period was 30 days. Safety index and efficacy index were measured for each treatment group.

Results:

1310 eyes treated with laser corneal refractive surgery (LASIK or surface ablation) and 94 eyes operated on with a phakic lens (ICL) were included in the study. Patient age was between 18 and 47 years. The mean preoperative SE was -7.24 D (range -5 to-10 D) + / - 1.18 D (SD) in the laser group, and -8.68 D (range -5.5 to -9.87 D) + / - 1.07 D (SD) in the ICL group. Snellen preoperative BSCVA was 0.62 (range 0.1 to 0.7) + / - 0.11 in the laser group, and 0.60 (range 0.2 to 0.7) + / - 0.10 in the ICL group. Postoperative SE was -0.30 D (range -2.38 D to +1.00) + / - 0.42 D in the laser group, and -0.19 D (range -1.37 D to +0.75) + / - 0.38 D in the ICL group. Postoperative uncorrected visual acuity (UCVA) was 0.76 (range 0.05 to 1) + / - 0.18 in the laser group, and 0.77 (range 0.45 to 1) + / - 0.15 in the ICL group, with a postoperative BSCVA of 0.80 (range 0.10 to 1) + / - 0.16 and 0.84 (range 0.58 to 1) + / - 0.12 respectively.

Conclusions:

Amblyopic eyes with moderate myopia undergoing corneal Laser refractive surgery or ICL implantation enjoy a statistically significant improvement in UCVA and BSCVA compared to preoperative figures. The ICL group performed significantly better than the laser group in terms of safety and efficiency.
