Abstract Details

Purpose:

The purpose of this study was to evaluate efficacy of selected methods for keratoconus treatment.

Setting:

1Ophthalmology Clinic - The Center of Postgraduate Medical Education - Warsaw, 2Ophthalmology Department – Medical University of Warsaw, 3LASER Eye Microsurgery Center – Warsaw

Methods:

Patients were recruited from Ophthalmology Clinic - The Centre of Postgraduate Medical Education) and LASER Eye Microsurgery Center – Warsaw 178 patients were divided into 4 groups: 1. CxL - 141, 2. INTACS - 30, 3. INTACS+CxL - 30, 4. PRK+CxL – 30 Procedures which were used in the research: CxL - cross-linking, INTACS - intracorneal rings, INTACS+CxL - intracorneal rings + cross-linking, PRK+CxL - photorefractive keratectomy + cross-linking Following parameters were evaluated: UCVA, BCVA, average keratometry, astigmatism, pachymetry. Examination intervals: before, 1, 3, 6, 12, 24 and 36 months after the procedures.

Results:

Group with the best results for UVA improvement is PRK+CxL in first 12 months and after 24 months ITACS+CxL ( +0,27 and +0,21; p<0,01) Group with the best results for BCVA improvement is INTACS in 24 months and INTACS+CxL after 36 months (+0,33 and +0,40; p<0,01) Group with the best astigmatism improvement is INTACS+CxL in first 6 months, INTACS after 12 months and INTACS+CxL after 24 months (-1,91; -2,80; -3,30; p<0,01) Group with the lowest pachymetry rate decrease is INTACS after 12 months and CxL after 24 months (-11,0 um; -37,0 um; p<0,01) Group with the highest average keratometry rate decrease is INTACS+CxL after 3 and 6 months and INTACS after 12 and 24 months.(-4,10; -4,30; p<0,01)

Conclusions:

Modern means of keratoconus treatment are good alternative to previous procedures and provide satisfying results in UVA, BCVA, astigmatism and keratometry parameters improvement.
