Same-day dual-interface technique for treating ametropic presbyopia with LASIK and corneal inlay implantation

Abstract Details

Purpose:

Assess the effectiveness of same-day laser vision correction and pocket implantation of a small-aperture corneal inlay to simultaneously treat refractive error and extend depth-of-focus in ametropic presbyopes.

Setting:

Optilase Laser Eye Clinics , Belfast & Dublin, Ireland

Methods:

Retrospective chart review of 162 patients implanted monocularly with a KAMRA corneal inlay (AcuFocus, Irvine, CA) in their non-dominant eye. The inlay is 5 µ thick and 3.8 mm in diameter with a 1.6 mm central opening. The inlay uses the concept of small-aperture optics to extend depth of focus of the eye. A traditional LASIK procedure was performed to achieve a post-op refractive target of -0.75D in the inlay eye and plano in the fellow eye. The inlay was then inserted into a femtosecond-laser created lamellar pocket at least 100 microns below the prior LASIK flap interface. The two surgical procedures were performed consecutively on the same day. Uncorrected distance, uncorrected near and best-corrected distance visual acuity (UCDVA, UCNVA and BCDVA respectively) outcomes were assessed at pre-op, 1 and 3 months post-op.

Results:

In the implanted eye, mean UCNVA improved 4 lines from 0.28 decimal ± 0.16 preop to 0.62 decimal ± 0.19 at one month (n=131); 0.64 decimal ± 0.19 at 3 months (n=57). Mean UCDVA remained at 20/25 from pre-op through to 3 months post-op. BCDVA also stayed consistent at 20/20 at all time points. Longer-term data will be presented.

Conclusions:

Early results demonstrate that this same-day dual-interface technique for corneal inlay implantation with laser vision correction is an effective surgical treatment option for ametropic presbyopia.
