Abstract Details

Purpose:

Visco-canalostomy and Canaloplasty (ab externo Schlemm’s Canal surgical procedures) have traditionally been considered as “non-penetrating surgery”, in which a “Trabeculo-Descemet’s Window” is kept intact, to provide a semi-permeable barrier between the anterior chamber and the “intra-scleral lake”; secondary Goniopuncture being performed with the Nd-YAG Laser as necessary. This has the disadvantage of maintaining some Trabeculo-Descemet’s resistance to outflow, in some patients, which may increase over time, requiring secondary YAG Goniopuncture. Primary Surgical Micropuncture eliminates the outflow resistance of the Trabeculo-Descemetic Window, at the time of surgery, reducing the need for secondary YAG Goniopuncture.

Methods:

The authors present the technique of “primary micropuncture of Descemet’s Window”, prior to tight suturing of the superficial scleral flap: a “one stage bleb-independent ab-externo micro-penetrating Schlemm’s Canal procedure” and discusses the history and ‘pros & cons’ of this surgical modification, together with the preliminary results.

Primary Surgical Micropuncture eliminates the outflow resistance of the Trabeculo-Descemetic Window, at the time of surgery, reducing the need for secondary YAG Goniopuncture.
