Abstract Details

Purpose:

To determine the safety and efficacy of a femtosecond laser which can be used for both cataract procedures and the creation of LASIK flaps. For femto-cataract procedures, both routine and challenging cases were assessed.

Setting:

Maxivision Eye Hospitals, Hyderabad, India

Methods:

A single centre study where the femtosecond laser (Victus, Bausch + Lomb) was used for performing anterior capsulotomy and lens fragmentation in cataract cases with the laser operating at 80 kHz. Accuracy of the anterior capsulotomy and reduction in effective phaco times (EPT) was assessed. Evaluation of the predictability and flap quality of the LASIK flap creation with the laser operating at 160kHz was also conducted.

Results:

Twenty eyes were evaluated for the flap quality and predictability. For an intended flap thickness of 120µm, the mean achieved flap thickness was 117.3 µm. Stromal bed quality very satisfactory with easy opening and good edge quality. For the cataract procedures, the anterior capsulotomies had excellent circularity, centration and diameter. A combination of radial and/or circular cuts were used for fragmenting the lens. Lens fragmentation is possible in hard grade 5 cataracts. Lens fragmentation prior to phacoemulsification resulted in a mean reduction in EPT of 50% across all cataract grades compared to our manual technique. The femtosecond laser was also used to create the anterior capsulotomy in a number of more challenging cases, including white soft cataracts, traumatic cataract, traumatic subluxated cataract with capsular calcification, paediatric and posterior polar cataract. All cases were successfully performed with no adverse events.

Conclusions:

A femtosecond laser with multi-procedural capabilities can safely and effectively be used for cataract procedures and LASIK flaps. This technology also has potential applications in more challenging cases.
