Purpose:

to review the incidence of capsulorhexis related complications in over 4000 femtosecond laser cataract procedures and discuss these results in reference to scanning electron microscopy imaging (SEM)

Methods:

A review of over 4000 laser cataract procedures at a single practice was undertaken to determine the incidence of anterior and posterior capsular tears. SEM imaging was undertaken in a selected cohort to identify possible structural defects that may increase the possibility of capsule-related complications. The initial 200 procedures were removed from the analysis due to the learning curve and early hard/software.

Results:

11/4000 patients had anterior and posterior capsular tears during surgery (6 anterior tears 0.15% and 5 posterior tears 0.13%) No posterior tears led to posterior lens dislocation requiring vitreo-retinal intervention. SEM imaging identified typical femtosecond laser edge appearance. Errant pulses were minimal.

Conclusions:

The incidence of femtosecond capsular tears is at least equal to, if not lower, than previously published references in manual surgical cohorts. Although an edge comparison with manual cases provides distinct differences, evidence does not suggest that these features facilitate an increase in capsular-related complications. SEM imaging in other laser units and literature will be discussed.
