Femtosecond laser-assisted cataract surgery: our initial experience

Abstract Details

Purpose:

To evaluate the safety and precision of creating an anterior capsulorrhexis, nucleus fragmentation & visual outcome using femtosecond laser in cataract surgery

Setting:

Centre for Sight Hospital,New Delhi,India

Methods:

Cataract surgery was performed using the Lensex femtosecond laser platform on 218 eyes of 189 patients aging 28-88 yrs between Feb 12 and May 13. Intraop evaluation of centration , diameter of CCC, nucleus fragmentation and effective phaco time was done. LRI was done for astigmatism upto 2D in 12 eyes. Post op vision was measured on 1day, 1week and 3weeks.

Results:

1. CCC was well centered with avg diameter 5.1mm in all cases. CCC was complete in 197 eyes, there were micro adhesions in 11 eyes, incomplete in 5 eyes and complete but not continuous in 5 eyes.Capsular tags causing extension ocured in 2 eyes. 2.Nuclear fragmentation was achieved in all eyes. 3. Posterior capsule was cut causing nucleus drop in one eye. 4. Effective phaco time was 0.12±0.04s for grade 2-3 cataracts and 0.23±0.02s for grade 4 cataracts. 5. Post op unaided vision of 6/6 was achieved on day1 in 156 and on 3wks in 178 of the 218 eyes.

Conclusions:

• The femtosecond laser offers to refine the present day cataract surgery to enhance visual outcomes. • Patients more likely to achieve 6/6 unaided vision sooner(71.5% on day 1 and 90.8% at 3 weeks) • Surgeons familiar with femto lasers have a shorter learning curve. • Further upgrades will fine tune the technique.
