Abstract Details

Purpose:

Evaluation of accuracy of depth and profile of lamellar dissection using the Gebauer stainless steel re-usable DSAEK-heads 500um and 550um cutting depth for the one pass technique.

Setting:

Queen Alexandra Hospital, Portsmouth, UK . Gebauer Research Facility, Neuhausen, Germany

Methods:

20 human donor corneas were salvaged from eye bank stock that had fallen out of the transplantation grade criteria. All were dissected by one surgeon in the manner used for ultrathin DSAEK preparation. Depending from the overall thickness of the cornea as measured by pachymetry, different Gebauer SLc System heads were used targeting a residual depth of <100um. Every cornea was measured with pachymetry 5 times before cutting. Measurements were made of the free cap using a gauge, and the donor cornea was maintained on the artificial chamber where measurements were taken using pachymetry and on the Anterior Segment OCT. Graft thickness was measured on the OCT at 5 points and the profile of the cut was analysed.

Results:

The remaining thickness of the donor was within +/- 30 um of the intended depth. No button holes were seen and the profile of the cut was regular with minimal asymmetry.

Conclusions:

The Gebauer SLc Reusable stainless steel heads offer a very reliable method for cutting ultra-thin DSAEK lenticules without the need for drying or using a two pass technique. The profile is more regular than other microkeratome systems allowing for potentially better vision with less irregular astigmatism. The technique is safe and reliable, and being a fully automated system, the results are user independent.
