Abstract Details

Purpose:

To evaluate how centration of a 1-piece diffractive multifocal IOL relates to angle kappa and early postoperative visual acuity.

Setting:

Empire Eye and Laser Center, Bakersfield, California, USA.

Methods:

Retrospective single-surgeon, observational case series. Digital mesopic pupil images were taken of 118 eyes before and after cataract extraction with implantation of a Tecnis Multifocal 1-piece IOL (ZMB00, Abbott Medical Optics). The locations of the subject-fixated coaxially-sighted corneal light reflex (CSCLR), the pupil center, and the IOL center were measured. IOL centration relative to the CSCLR and the pupil center was correlated to angle kappa and uncorrected distance vision acuity on postoperative day 1. Intraoperatively, the IOLs were aligned toward the CSCLR.

Results:

Mean angle kappa was 0.26 ± 0.12 (range 0.03 to 0.56) mm. On postoperative day 1, the IOLs were decentered a mean 0.15 ± 0.09 mm from the CSCLR and 0.21 ± 0.11 mm from the pupil center. The degree of IOL decentration from the CSCLR and the pupil center was positively correlated with angle kappa (R = 0.27 and 0.48, respectively). Although weak, uncorrected visual acuity was positively correlated with CSCLR alignment (R = 0.07) and negatively correlated with pupil center alignment (R = -0.05) and angle kappa (R = -0.07).

Conclusions:

Diffractive multifocal IOLs can be accurately centered on the CSCLR. Good centration on the CSCLR appears to be correlated with better early uncorrected visual acuity. Although small overall, IOL decentration tends to be greater in eyes with larger angle kappas.
