Abstract Details

Purpose:

Silicone oil tamponade in vitrectomized eyes will cause a hyperopic shift. BUCVA will drop and other problems like anisometropia, aniseiconia, reduced stereopsis, diplopia etc. might occur. We like to introduce a new technique to reduce refractive complications induced by silicone oil tamponade.

Methods:

Since January 2013 we are using a new option for selected cases to solve these problems. If pars plana vitrectomy with silicone oil tamponade is performed in a pseudophakic eye or the surgery is carried out in combination with phacoemulsification and implantation of an IOL we additionally implant a so-called add-on IOL (1stQ A4SW00 +6.0dpt, Medicontur, Hungary) in the cilliary sulcus to treat hyperopic shift. Indication for silicone oil tamponade were retinal detachment, severe proliferative diabetic retinopathy, recurrent vitreous hemorrhage and macular translocation.

Results:

There was no complication related to the implantation of an additional IOL into the cilliary sulcus and all eyes had an improved postoperative refraction compared to eyes without this solution. Most eyes were on the mild myopic side. Only one eye had a postoperative refraction of -3 diopters. The add-on IOL was well cetered in all eyes and no silicone oil prolaps or posterior synechiae occured.

Conclusions:

After more then one year experience with this new technique we believe that hyperopic shift after vitrectomy with silicone oil tamponade can be treated by implanting an add-on IOL with +6 dopter power. The special design oft he 1stQ A4SW00 (Medicontur, Hungary) prevents complications like decentration, posterior synechiae, interlenticular secondary cataract formation and iris capture. Not only our own perception but feed back from patients and referring doctors was very positive.
