Abstract Details

Purpose:

To evaluate & ascertain the advantage of this technique using both anterior chamber & posterior chamber skills & it's influence on intraoperative & postoperative complications, the visual outcome in phacoemulsification & trans pupillary silicone oil removal through planned posterior curvilinear circular capsulorhexis

Setting:

Methods:

90 patients who had previously operated for retinal detachment by the same surgeon using triport ppv + silicone oil injection underwent combined phacoemulsification & trans posterior ccc silicone oil removal by the same surgeon, 49 of them are females & 41 males. In 35 of the patients the cause of RD was a break (rhegmatogenous RD varying from single small to multiple & giant one), in 49 tractional RD of PDR, in 5 patients intraocular metallic FB & traumatic vitreous Hrge & macular hole with retinal detachment in one patient. The mean interval between PPV & PE with IOL implantation is 7 months, the mean interval between PE & final exam 12 months, the mean age 56 years.

Results:

Intraoperatively, i didn’t face any problems except in 6 patients there was posterior capsular plaque (fibrosis) where the post CCC was assisted by peeling of the plaques & the use of scissors, posterior capsular tear & extension of the posterior CCC occurred in 3 patients & in one of them the IOL implanted on the anterior capsule & in the sulcus. postoperatively, the BCVA improved in 95.6%, sterile fibrinous reaction in A/C occurred in 3 cases, no IOP rise except in 5 patients who had preoperative silicone induced high IOP, 4 of them controlled by medications postoperatively & now they are under control without any medications, 1 patient still on treatment, in all cases we didn’t see any corneal decompensation, retinal redetachmant occurred in only 1 patients ( 1.1%) with a previous inferior RD plus severe PVR which is then reoperated with silicone oil reinjection .

Conclusions:

Combined phacoemulsification & trans-posterior CCC silicone oil removal through the same small corneal incision was found to be safe & the method of choice in cases of cataract with previous silicone oil injection with stable retina & in reducing the intraoperative & postoperative complications. . Combined phacoemulsification & Trans posterior CCC silicone oil removal through the same small corneal incision provides visual rehabilitation & avoid the risk of multiple surgeries. . retinal pathology influence the visual outcome
