Evaluation of postoperative rotation of toric intraocular IOLs implanted: influence on visual acuity, wavefront aberrations and optical quality of vision

Abstract Details

Purpose:

The aim of this study was to evaluate the postoperative rotation of toric intraocular lens (IOL) and to correlate with visual acuity, optical quality of vision (Point Strehl ratio, PSF and Modulation Transfer Function, MTF) and wavefront aberrations.

Setting:

University Eye Clinic of Trieste

Methods:

We conducted a prospective study of 53 eyes of 41 patients (mean age: 76.5 years old) affected by cataract and moderate corneal astigmatism (1,5 D - 3 D). All surgeries were performed with the same surgical technique and the same surgeon. (DT) Toric IOL rotation, uncorrected and corrected visual acuity (UCVA), best corrected visual acuity (BCVA), high grade aberrations (HOA), PSF and MTF were evaluated over a 6 months follow up period. OPD Scan III (Nidek) was used for the valuation in the follow-up period.

Results:

The mean UCVA improved significantly from 0,45±0,3 logMAR to 0,13±0,15 logMAR(p<0.01) and the mean BCVA improved significantly from 0,28±0,2 logMAR to 0,048±0,21 logMAR(p<0.01). The mean toric IOL rotation was 6, 98 ± 4, 53°. The rotation of IOLs demonstrated a significant statistic correlation with visual acuity (p<0.01) and MTF (p<0.01), but there was not showed any correlation with PSF (p=0.2) and high order aberrations (p=0.1) and coma-like aberrations (p=0.3).

Conclusions:

Cataract surgery with implantation of toric IOL is an effective technique to treat cataract and moderate astigmatism and it resulted in good postoperative optical quality. Postoperative rotation of the toric IOL represents an important data to consider in the follow-up to improved visual acuity and optical quality of vision.
