Why the ReLEx SMILE is the state-of-the-art refractive surgery to treat myopia

Abstract Details

Purpose:

Reporting the visual outcomes and describe the surgical technique and the evolution of the ReLEx SMILE indications in the treatment of myopia.

Setting:

Espace Nouvelle Vision refractive surgery center and Quinze-Vingts Ophthalmology National Hospital, Paris, France.

Methods:

Since 2011, 400 eyes underwent a refractive lenticule extraction surgery to treat their myopia or myopic astigmatism. The mean patient’s age was 32 years old. The pre operative refraction ranged from -1, 00 to -10, 00 diopters with no cylinder up to 2, 00 diopters. The patients were followed up at 1 day, 1 month, 3 months and 6 months after surgery. The uncorrected distance visual acuity (UDVA), the post operative refraction, the interface and the lacrymal evaluation and the per operative laser parameters were reported.

Results:

At 6 months, 96% of patients raised 20/20 of UDVA and 100% raised 25/20 or more and the mean spherical equivalent was -0.42 diopters. The incision ranged from 2.6 to 3.5 millimeters and the caps were created between 105 to 160 microns depth. The thinnest lenticule extracted measured 42 microns. The OCT spectralis showed normal corneal structures. The mean score at the Ocular Surface Disease Index questionnaire was 7, 55%. The mean tear osmolarity measured was 300,36 mOsm/L, the mean result at the Schirmer test was 17,28mm, for the Tear Break Up time test , it was 7 seconds and the mean esthesiometric result was 5,78cm . The ReLEx SMILE represented 53% of the myopic laser refractive treatments during the first three months of the study while it represents 90% during the last three months.

Conclusions:

The ReLEx SMILE seems to be an alternative to LASIK to prevent the post operative dryness. The visual outcomes are satisfying and the new possibilities offered by The ReLEx SMILE technique surgery should turn it into the first refractive treatment choice for myopia. Regarding to the results obtained an evolution of the ReLEx SMILE technique for the treatment of presbyopia and hyperopia is expected.
