Abstract Details

Purpose:

To evaluate efficacy and outcome of viscosealing of incisions made during pediatric cataract surgery

Setting:

Retrospective case series of children aged less than 10 years who underwent congenital cataract surgery in the paediatric ophthalmology clinic of a tertiary eye care centre in India

Methods:

After institutional ethical board clearance, analysis of all children with congenital cataract who underwent cataract surgery with intraocular lens implantation from January 2013 to December 2013 was done. The patients were divided into two groups: Group 1 with sealing of the main as well as the side port with 10-0 polyglactin suture and Group 2 included patients where sealing of the sideports was done with 1.45% sodium hyaluronate. In all the patients’ phacoaspiration with primary posterior capsulorhexis and anterior vitrectomy was done. Hospital records of the intraoperative and post operative complications on subsequent examinations under anesthesia in these patients were reviewed.

Results:

50 paediatric eyes undergoing phacoaspiration with intraocular lens implantation were included (25 in each group). Mean age at surgery was 9.48 +/- 7.58 months in group 1and 11.75 +/- 8.33 months in Group 2. Major complications encountered in group 1 were hyphema in 6 eyes, irregular pupil in 4 eyes, iridolenticular adhesions in 8 eyes and vascularisation of the ports in 6 eyes post operatively. The major complications in group 2 where the ports were viscosealed, were irregular pupil in 7 eyes, iridolenticular adhesions in 2 eyes, and shallow anterior chamber in 4 eyes post operatively. Most of the complications in group 2 were seen in children < 6 months of age (36.3%). At six weeks post operatively the mean spherical equivalent was 1.08 + 2.27 D in group 1 and 1.02 + 2.26 D in group 2(p=0.06).

Conclusions:

Viscosealing is an effective way to close sideports in children above 6 months age. It is equally efficacious to sutures and does away with suture related problems and need for second time general anesthesia.Financial disclosures: none
