Manual versus femtosecond laser keratoplasties: astigmatism comparison

Abstract Details

Purpose:

To assess the astigmatic changes between before and after complete suture removal (ROGS) in eyes with keratoconus undergoing keratoplasty with either a manual technique or using the femtosecond laser.

Setting:

Moorfields Eye Hospital Dubai

Methods:

Retrospective comparative consecutive case series involving 82 eyes which had undergone manual versus 68 eyes which had undergone IntraLase femtosecond laser keratoplasty.

Results:

No statistically significant differences were detected in the mean arithmetic astigmatism magnitude either before or after removal or graft sutures (ROGS) between the groups. Mean astigmatism before ROGS: Manual v Femto PK (4.65±2.17; 6.32±2.91 D; p: 0.15); Manual v Femto DALK (8.50±4.50; 4.25±1.97 D; p: 0.20); Femto PK v Femto DALK (6.32±2.91; 4.25±1.97 D; p: 0.14). Mean astigmatism after ROGS: Manual v Femto PK (5.17±2.60; 5.57±2.18 D; p: 0.68); Manual v Femto DALK (6.62±3.45; 6.67±3.50 D; p: 0.99); Femto PK v Femto DALK (5.57±2.18; 6.67±3.50 D; p: 0.54). The ROGS-induced Vector computed astigmatism was greatest in the Manual DALK group (5.49±1.82 D) and lowest in the Femto DALK group (3.18±1.87 D, p>0.05).

Conclusions:

Manual and Femtosecond laser PK and DALK induced similar magnitudes of postoperative and post-ROGS astigmatism and the use of the femtosecond laser was not associated with lower astigmatism.
