Purpose:

Compare global visual results after phacoemulsification and implantation of two different multifocal IOLs

Setting:

Ophthalmology Department - Centro Hospitalar do Porto – Hospital Santo António (HSA) - Portugal

Methods:

A prospective, nonrandomized comparative trial of 30 patients (60 eyes) aged 44-70 (mean age 57.2±6.03) divided into two equal groups of MIOLs implantation: Acrysof Restor (SA60D3, Alcon Laboratories, USA) and Tecnis MF (ZM900, Abbott Medical Optics). The patient selection criteria included no other eye disease besides cataract, corneal astigmatism of 1.0 diopters or less. In each case, the patients were scheduled to have standard phacoemulsification surgery with IOL implantation in the bag. The postoperative target was emmetropia. The minimum considered follow-up was 3 months. The following parameters were assessed: slit-lamp examination, uncorrected and best spectacle-corrected far, intermediate and near visual acuity, objective refraction, intraocular pressure, contrast sensivity, defocusing curves, subjective satisfaction with vision using VF-14 survey, perception of halos/glare, pupil diameter, spectacle independence and refractive stability.

Results:

After surgery all eyes were within ±1.0 D of target refraction. The mean uncorrected distance visual acuity improved in all cases. The postoperative BCVA was better than 20/30 at third month in all cases. Three months postoperatively, the mean uncorrected distance visual acuity (UCDVA) ± SD in the Restor Vs Tecnis group was 0.75± 2.13 and 0.80 ±1.73, respectively. All results were significantly better compare to preoperative values (p<0.01) but did not differ between the two groups (p>0.05). In patients with residual refraction, we proceeded to a secondary correction laser procedure, obtaining 100% of spectacle independence. At the final visit 92 % in the Restor group and 93 % in Tecnis group achieved J1 (Jaeger standard). 93 % and 95 % with bilaterally implanted Restor and Tecnis MFIOLs were spectacle independent .32 % of patients with implanted Acrysof Restor IOL and 20% with Tecnis IOL reported little or moderate halo or glare. The patients satisfaction after the procedure was excellent; the scoring of VF 14 ranged from 70-100% with an average of 91 %. No patient was dissatisfied with results of surgery.

Conclusions:

The implantation of tested multifocal intraocular lenses provides good near, intermediate and distance vision. We do not observe any statistically significance differences between the tested lenses with regard to best corrected distance visual acuity, presence of glare and halo, as well as satisfaction with vision.
