Abstract Details
Purpose:
To analyze the impact of different error contributions to the IOL
position prediction and to the refractive prediction in very short eyes.
Setting:
University eye hospital Mainz, Germany, Eye clinic
Castrop-Rauxel, Germany
Methods:
IOL position was predicted in 28 eyes shorter than 22.5mm with Phacooptics and OKULIX raytracing based on preoperative
Lenstar (Haag-Streit) axial eye length, corneal thickness and crystalline lens position and thickness measurements prior to complication-free cataract
surgery. Postoperatively, IOL position was measured with the Lenstar again.
For all three methods the refractive prediction errors based on the predicted or measured IOL positions were calculated.
Results:
The 2 predicted positions differ from each other only by a constant
offset depending on the average IOL position assumptions of either theoretical method, as do accordingly the refractive predictions. The measured position
data are in between the predictions of these 2 methods. However, the scattering (e.g. standard deviation) of the refractive predictions based on the measured data is about 50% higher than that of the 2 predictions.
Conclusions:
Because stochastical measurement errors of the observed amount are quite unlikely, also IOL mislabelling errors, even if in accordance with ISO 11979, may have a significant
impact on IOL refractive prediction accuracy, particularly in high power IOLs.
