Abstract Details

Purpose:

To determine the stabilization time of automated refraction, keratometry, ocular biometric parametres and central corneal thickness (CCT) after cataract surgery to allow prescription of satisfactory spectacles.

Setting:

Hitit University, Faculty of Medicine, Department of Ophthalmology, Corum. Yuzuncu Yil University, Faculty of Medicine, Department of Ophthalmology, Van. Canakkale State Hospital, Ophthalmology Clinic, Canakkale.

Methods:

This was a prospective, randomized, case series study of 62 consecutive eyes undergoing uncomplicated cataract surgery. Automated refraction, keratometry, ocular biometric parametres and central corneal thickness (CCT) were made at baseline before cataract surgery and on follow-up visits at 1 day, 1,2 and 4 weeks afterward. Four different surgeons performed the phacoemulsification using a similar surgical technique. The stabilization of ocular parametres were analyzed by analysis of variance (ANOVA) for repeated measure. Duncan’s multiple compare test was performed to determine the different groups. The data were analyzed using the Statistical Procedures for the Social Sciences (SPSS for Windows software, version 15.0).

Results:

The study was composed of 62 eyes of 62 patients with a mean age of 66±12.38 (range, 40 to 84 years). The mean CCT was 535,69 µm at baseline visit. It was 577,48 at 1 day, 561,98 at 1 week, 542,14 at 2 weeks, 531,84 at 1 month after cataract surgery, respectively. There was no statistical different in postsurgical spherical refraction error and mean keratometric measurements among all visits. The mean keratometric corneal astigmatism was 0.71 at baseline visit. It was 0.98 at 1 day, 0.94 at 1 week, 0.74 at 2 weeks, 0.73 at 1 month after cataract surgery, respectively. The mean anterior chamber depth was 3.09 mm at baseline visit. It was 3.84 at 1 day, 3.91 at 1 week, 3.64 at 2 weeks, 3.78 at 1 month after surgery, respectively. The mean lens thickness was 4.66 mm at baseline visit. It was 4.82 at 1 day, 4.77 at 1 week, 4.39 at 2 weeks, 4.97 at 1 month after surgery, respectively. The mean vitreus length in biometry was 15.87 at baseline visit. It was 14.81 at 1 day, 14.82 at 1 week, 14.88 at 2 weeks, 14.71 at 1 month after surgery, respectively.

Conclusions:

The stabilization of CCT was provided after the second postsurgical week. One day after surgery, the cornea were 7.98 % thicker than at baseline. The stabilization of cylindrical refraction error was provided after the first postsurgical week.The stabilization of mean keratometric corneal astigmatism in keratometry was provided after the second postsurgical week. The stabilization of mean anterior chamber depth, lens thickness and vitreus length in biometry were provided after the second postsurgical week. We found statistically significant decreases in corneal swelling until the second postsurgical week. We found that automated refraction stabilized 1 week after surgery, and changed minimally between the first week and the first month after cataract surgery. Corneal swelling, ocular biometric parametres and keratometric corneal astigmatism become stable after 2 weeks. It may be possible to prescribe spectacles beginning 1 week for most patients after uneventful phacoemulsification cataract surgery.
