Abstract Details
Purpose:
To evaluate long-term follow-up of full-ring intracorneal implant (MyoRing) for the management of keratoconus.
Setting:
Ophthalmology Department, Bina Eye Hospital, Tehran,Iran
Methods:
Of the total 69 patients contacted, in eight patients (eight eyes), Myoring were removed three to 12 months after implantation. From the remaining 61 patients , twenty four patients (24 eyes) were unable to attend follow-up appointments because of work and family commitments and were, therefore, excluded from the present study.The remaining 37 patients (40 eyes) (19 male and 18 female) with moderate and severe keratoconus ages 17 to 45 years who underwent myoring(Dioptex GmbH) implantation using a PocketMaker Microkeratome and attend 3 years of follow-up appointments were included in this study . The uncorrected visual acuity (UCVA) , best spectacle-corrected visual acuity (BSCVA), refraction, and keratometry (K) readings were measured and evaluated clinically and statistically preoperatively and 3 years postoperatively .
Results:
No intraoperative complications were demonstrated in this series of patients .Three years postoperatively, there was significant improvement in UCVA, BSCVA, K readings, spherical equivalent (SE), central corneal thickness (CCT) and manifest sphere and cylinder (P<0.05). Myoring implantation significantly increased UCVA from 1.14 (0.27) logMAR to 0.30 (0.21) logMAR (P=0.001), BSCVA from 0.52 (0.23) logMAR to 0.18 (0.12) logMAR (P=0.001), and decreased the spherical equivalent by 4.35 diopters (D) and the average keratometric values (K value) by 2.34 D (P=0.001) .One eye maintained the preoperation best spectacle corrected visual acuity (BSCVA) ,13 eyes achived 3 to 7 lines of BSCVA, whereas the rest of the eyes experienced a gain of 7 up to 13 lines. Meanwhile , 75.70% of patients were satisfied after 3 years of implantation and 64.90% of patients agree to implant Mayoring for other eye.
Conclusions:
After three years, Myoring implantation improved UCVA, BSCVA, and refraction in the majority of the keratoconus patients. Myoring implantation using PocketMaker Microkeratome is a minimally invasive procedure for improving visual acuity (both UCVA and BSCVA) in keratoconic patients
