Passive versus active infusion in phacoemulsification: a comparative study

Purpose:
To evaluate and compare the safety and efficacy of passive and active infusion in uncomplicated coaxial phacoemulsification.
Setting:
TOBB ETU Hospital,Ankara Turkey
Methods:
A coaxial phacoemulsification of 25 cases with active infusion (group I) and 25 cases with passive infusion (group II)were done. Optical coherence tomography was used to analyze central macular thickness (CMT), Heidelberg Retinal Tomografi was used analyze peripapillary retinal nerve fiber layer (RNFL) thickness. Measurements of passive and active infusion groups, taken 1, 30, and 90 days after coaxial phacoemulsification were compared with the pre-operative values of both groups. Surgical time, phaco energy, irrigation fluid volume, surgical ease, number of surges observed during removal of divided nuclear pieces, complications, and visual gain in the 2 groups were compared.
Results:
The surgery was easier with active infusion. Almost no surge occurred in the active infusion group, anterior chamber control was much better, surgical time was significantly shorter. Total amount of infusion volume was significantly lower with the active infusion. The rate of visual gain, CMT, peripapillary RNFL thickness, phaco time, and amount of phaco energy were comparable in the two groups.
Conclusions:
Active infusion was safe and effective in controlling surge and increased the safety, ease, and speed of coaxial phacoemulsification.
