Purpose:
To investigate the clinical outcome of different lens fragmentation patterns in femtosecond laser-assisted cataract surgery

Methods:
110 eyes of 110 patients was undergone cataract surgery between December 2013 and March 2014 and we performed retrospective chart review about them. We conducted cataract surgery using the Catalys Precision Laser System(Abbott Medical Optics Inc.) or conventional manual technique. Among them, 35 eyes were undergone lens fragmentation on the pattern of quadrants softened(Quadrant) and 25 eyes were undergone lens fragmentation on the pattern of sextant. The rest 50 eyes were treated with the conventional manual technique. The data on patient demographics, total used Balanced Salt Solution volume and used BSS volume during phacoemulsification, I&A, viscoelastic removal procedure, effective phacoemulsification time, cumulative dissipated energy(CDE), refractive outcome and best corrected visual acuity, difference between the preoperative and postoperative endothelial cell density were collected.
Results:
There was a statistically significant difference in the mean phacoemulsification time between the three groups as follows: 21.72 ± 5.32 seconds in the quadrant group versus 38.21 ± 4.66 seconds in the sextant group versus 30.12 ± 5.5 seconds in the conventional group(P=.024); Post hoc analysis showed significant difference between the quadrant group and the other group(p=0.012, 0.038). The mean balanced salt solution volume during I&A was 19 ± 16.4, 22.6 ± 7.7, 14.23 ± 5.9 in each group and there was significant difference between the three group. Post hoc analysis showed significant difference between the sextant group and conventional group(p=.038). There were no significant difference between the three groups in the CDE, refractive outcome, difference between the preoperative and postoperative endothelial cell density.
Conclusions:
Quadrant lens fragmentation using femtosecond laser results in significantly lower effective phacoemulsification time when compared to the sextant fragmentation pattern and conventional manual technique. Sextant lens fragmentation pattern increases the volume of irrigation during I&A.
