Abstract Details
Purpose:
To assess the effect of diagnosis/host pathology on the visual outcome
after Descemet Stripping Endothelial Keratoplasty.
Setting:
Southampton University Hospitals NHS Trust
Methods:
This prospective analysis included 78 consecutive cases
of 74 patients that underwent manually dissected
DSEK cases over a period of 36 months.
Serial postoperative measurements of central graft,
and total corneal, thicknesses were made using anterior
segment optical coherence tomography.
LogMar visual acuity was also recorded at each
post-operative visit. The effect of diagnosis (host pathology), time and graft thickness on best corrected visual acuity was analysed using regression tree analysis. The statistical packages used were Statistica and SPSS
Results:
72% of our cases suffered from Fuch’s endothelial dystrophy. Bullous keratopathy represented 22% of the host pathology and 6% of the cases were DSEK redos after failed previous DSEK. Regression tree analysis and ranking calculation showed strong effect (predictor importance of 1.00) for graft thickness, mild effect (predictor importance of 0.39) for time and only a minimal effect (predictor importance of 0.09) for diagnosis.
Conclusions:
There was no difference in BCVA between Fuch's endothelial dystrophy, Bullous keratopathy and DSEK redos. Different host pathologies do not appear to show a difference in the visual outcome in DSEK.
