Purpose:
The AA shows a simple way to manage subluxated lenses in cataract surgery. This method is effective and functional till a 180° zonular dialisys
Setting:

Methods:
34 phaco surgeries in subluxated lenses have been performed in our department in the last 3 years. The subluxated lenses were the result of pseudoexfoliation syndrome, trauma, Marfan syndrome. The surgery follows these steps: local anesthesia, trocars insertion, capsulorexis, capsular hooks, phaco surgery, tension ring and IOL in the bag.
Results:
31 surgeries had the IOL in the bag. Three cases had capsular posterior brake and nuclear drop into the vitreous; we performed vitrectomy and scleral fixation lens. Mean best corrected visual acuity has been 18/20, no retinal detachment, no vitreous in anterior chamber. Previous glaucoma affected eyes had no worsening of intraocular pressure.
Conclusions:
Subluxated lenses are a very interesting challenge for ophthalmic surgeon. We can apply this method as a routine surgery for these complicated cases.
