Abstract Details

Purpose:

To evaluate the benefit of toric intra-ocular lens (IOL) implants in cataract patients with keratoconus.

Setting:

Ein-Tal Medical Centre, Tel Aviv, Israel

Methods:

Patients representing with stable keratoconus and a regular component of astigmatism underwent cataract surgery and a toric IOL implantation. Visual acuity, keratometry, topographic and tomographic corneal displays were evaluated prior to surgery. Main outcome measures were postoperative visual acuity and subjective residual astigmatism.

Results:

Cataract surgery with a toric IOL implantation was performed on 12 eyes of 10 patients with stable keratoconus. Uncorrected and best-corrected distance visual acuities improved in all patients following surgery, changing from 0.95 preoperatively to 0.35 logMAR after surgery and from 0.35 to 0.18 logMAR, respectively. Anterior corneal astigmatism ranged from 1.86 to 7.48D, accompanied by high posterior astigmatism up to 1.8D. Postoperative subjective astigmatism was 1.23±1.0 (range 0 to 2.5D).

Conclusions:

Toric IOL lenses may reduce ocular astigmatism and improve post operative visual acuity in KC patients with a regular astigmatic component. Under correction of the corneal astigmatism is recommended due to the irregular astigmatic pattern and commonly high posterior cornea astigmatism.
