Purpose:
To analyse factors affecting postoperative visual recovery after Relex® SMILE
Setting:

Methods:
In this prospective, comparative, non randomized study, 197 eyes of 101 patients with myopia or myopic astigmatism of -4.16 ± 2.09 diopters (D) who underwent Relex® Smile were analysed with manifest refraction, uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA), topography, aberrometry, keratometry, pachymetry, preoperatively as well as 1 week, 1 month and 3 months postoperatively. Two groups were formed based on the CDVA at 1 month postoperatively: CDVA <20/20 Snellen (<0.00 logMAR) (group 1), n = 16; and CDVA ≥ 20/20 Snellen (≥0.00 logMAR) (goupe 2) n= 179.
Results:
There was no statistically significant difference between the two groups in preoperative for : mean manifest refraction spherical equivalent (MRSE), cylinder, keratometry, pachymetry, topography and modulation transfer function (MTF). At 1 month postoperatively, (MRSE) was similar -0.24 ± 0.49 for the groupe 1 and -0.02±0.59 for the groupe 2 (p=0.07). At 1 month postoperatively, the MTF values were statistically inferior for the group 1. At 1 month postoperatively, corneal asphericity factor (Q) of the two groups were statistically different (p<0.05) with Q= -0,30+/- 0,55 for group 1 and Q = 0,13+/- 0,44 for group 2. At 3 months postoperatively, these differences were not observed anymore, and we noticed an improved CDVA in both groups.
Conclusions:
At 1 month, for patients with slow vision recovery, we found a central corneal steepening affecting visual quality without influence on manifest refraction. At 3 months, all patients recovered normal post operative corneal shape and normal visual quality. Complementary studies to understand causes of this transient corneal steepening are needed.
