Two-step approach for iris reconstruction in patients with congenital iris coloboma and cataract

Abstract Details

Purpose:

To assess the results of lens replacement surgery in patients with congenital iris coloboma.

Setting:

S.Fyodorov Eye Microsurgery Complex

Methods:

11 patients (17 eyes) with cataracts of various densities underwent clear corneal phacoemulsification and IOL implantation. As a first step of pupil reconstruction iridoplasty with Siepser sliding knot sutures was done at the time of surgery. The 2nd second step was performed from 3 to 6 months after the initial surgery – for that purpose Argon laser applications in midperiphery of the iris were used to correct pupil size and position.

Results:

In 3 cases modified CTR was sutured to the sclera at 6 o’clock position. The most common intraoiperative complication was mild iris bleeding (6 eyes). No major complications were observed after the surgery. Preoperative BCVA (Snellen) 0.08 +/- 0.06 improved to 0.69 +/- 0.21. Mean follow-up period was 21 months. After the 2nd step improvement in BCVA was achieved (0.1+/_0.04) due to slight reduction of astigmatism resulting from better pupil alignment with the center of the cornea.

Conclusions:

In patients with cataract and congenital iris coloboma, two-step approach consisting of phacoemulsification, in-the-bag IOL implantation iris coloboma suturing, followed by laser pupilloplasty provided favorable functional and cosmetic results.
