Abstract Details
Purpose:
To evaluate the efficacy and safety of placement of the new iStent GTS-400 combined with phacoemulsification in patients with coexistent cataract and open-angle glaucoma or ocular hypertension (OHT).
Setting:
Ophthalmology Department. Hospital Clinico San Carlos, Madrid. Spain.
Methods:
A prospective, non-comparative, uncontrolled, non-randomized, interventional case series study was carried out in subjects with coexistent mild or moderate open-angle glaucoma (including pseudoexfoliative) or OHT and cataract who underwent phacoemulsification and intraocular lens implantation along with ab-interno implantation of two Glaukos GTS-400 iStents devices. Efficacy outcome measures were intraocular pressure (IOP) and the number of antiglaucoma medications used pre and postoperatively. Safety outcome measures were complications and best corrected visual acuity (BCVA). Mean follow-up was 31.2±7.18 months.
Results:
The 20 patients enrolled were aged 47 to 89 years (mean age 75.1±8.6). Mean baseline IOP was 19.95±3.71 mmHg with medication and 26±3.11 mmHg without medication. The mean final IOP was 16.8±2.02, determining a final IOP decrease of 34.66%, or 9.21±3.58 mmHg (P<0.001), from baseline washout IOP, and an IOP drop of 14.42%, or 3.15±3.9 mmHg (P=0.002) from medicated baseline IOP. The mean number of pressure-lowering medications fell from 1.25 ±0.64 to 0.65±0.67 (P=0.007). In 45% of patients, no antiglaucoma medications were required at the end of follow-up. Mean BCVA improved significantly from 0.4±0.12 to 0.68±0.22 (P<0.001). No complications of surgery were observed.
Conclusions:
Combined cataract surgery with implantation of Glaukos GTS-400 iStent seems to be an effective and safe procedure to treat coexistent open-angle glaucoma or OHT and cataract.
