Abstract Details

Purpose:

Design and evaluate an intraocular lens (IOL) that would preclude negative dysphotopsia (ND), allow for any optic concept (toric, multifocal, aspheric, etc.), allow for in the bag placement, allow for standard haptic design, and be consistent with routine phacoemulsification technique.

Setting:

Private Practice – Los Angeles, CA and Ruhr University Eye Hospital, Bochum Germany

Methods:

Based on prior clinical studies and ray tracing investigations, a hydrophilic acrylic IOL was designed to create a circumferential groove at the periphery of the anterior surface to capture the anterior capsulotomy. The chief purpose is to allow a portion of the optic to override the anterior capsule as this is the key element to reduce ND. 5 Patients had cataract surgery by femtosecond laser technique and phacoemulsification, assuring a well placed and sized anterior capsulotomy. OCT confirmed the relationship of the IOL/bag complex. Patients were evaluated 3 months following surgery with regard to VA, IOL centration, ocular inflammation, iris chafe and symptoms of either negative or positive dysphotopsia.

Results:

All 5 patients were free of any form of dysphotopsia at any time after surgery. There were no signs of iris chafe or ocular inflammation and all posterior capsules appeared clear. VA was consistent with presurgical evaluation as two of the 5 patients had dry ARMD.

Conclusions:

Based on this “proof on concept” preliminary investigation, the anti-dysphotopic IOL appears to match expectations and a broader clinical investigation can be pursued.
