Central toxic keratopathy: ache for the LASIK surgeon

Abstract Details



Purpose:

To report 5 cases of Central Toxic Keratopathy (CTK) seen in my practice.

Setting:

Tertiary care, private hospital in urban city of North India.

Methods:

Records of 5 patients who had developed opacification after LASIK were retrieved and followed up till February 2014.

Results:

7 eyes of 5 patients were identified to have developed CTK from January 2011 to February 2014. It was not necessarily in the visual axis, but all showed a mild hyperopic shift. All the cases were identified on the 1 week followup and none had associated DLK. All the 7 eyes cleared up over a period of 1 month to 12 months. Flap of 1 eye of the first patient who developed CTK was lifted and washed, but no difference in final outcome of the 2 eyes was noted in terms of the attainment of corneal clarity. However, the eye where interface was washed, has a higher hyperopic shift compared to the other eye. The flap in the area of the opacification was very thin when lifted and had to be replaced very carefully as it was very friable. None of the patients have had an enhancement to date and no eye has had a loss in BCVA.

Conclusions:

Literature search showed this entity to be a non inflammatory opacification with a hyperopic shift and our cases too fit in the same picture. It is an ache both for the patient as well as the surgeon as it causes a residual refractive error, more frequent followup and possibility of enhancement. FINANCIAL INTEREST: NONE
