Pressure-induced interlamellar stromal keratitis: a rare complication after LASIK

Abstract Details

Purpose:

To describe 4 cases of PISK (Pressure-induced Interlamellar Stromal Keratitis) with symptoms similar to those of Diffuse Lamellar Keratitis (DLK)

Setting:

Jakarta Eye Center (JEC), Indonesia

Methods:

A total of 2,547 eyes were treated with LASIK at JEC from January 1 to December 31, 2012; and 7 eyes (4 patients) came with discomfort and blurred vision within 3-4 weeks after LASIK. All of them with elevated IOP and diffuse haze corneas similar to DLK.

Results:

Three patients were falsely treated with steroid eye drops in the first few days, but after the conditioned worsed and immediately stopped the steroids and received pressure-lowering medications. All of cases were dramatically improved and regained their normal visual acuity except for 1 eye. For this particular paitent, IOP on the left eye was difficult to control despite of glaucoma medications so trabeculectomy was done. At final visit, IOP was under control without medication, but left the patient with permanent visual damage and corneal scarring.

Conclusions:

PISK is a rare complication of LASIK that resembles the DLK. Majority of cases will improve with cessation of steroids and glaucoma medications, but delay in diagnosing this conditions could result in permanent visual damage. FINANCIAL INTEREST: NONE
