Subjective satisfaction with near vision after bilateral refractive lens exchange: multifocal versus trifocal intraocular lenses

Purpose:
Cohort study was performed to assess subjective satisfaction with near vision functions after bilateral refractive lens exchange (RLE) and to compare subjective experience with near and intermediate vision in patients with trifocal and multifocal intraocular lens (IOL).
Setting:
European Eye Clinic Lexum Brno, Czech Republic
Methods:
Study group consisted of 40 patients (mean age 54.6) who underwent RLE with bilateral implantation of either multifocal or trifocal IOLs. The mean preoperative refractive error was 0.77 diopters (D) (range: -9.75D to +6.25D). Decimal monocular and binocular uncorrected distance visual acuity (UDVA) and uncorrected near visual acuity (UNVA) were measured 3 months postoperatively. Patient satisfaction with uncorrected near vision was evaluated using standardized 10-item questionnaire (Near Activity Visual Questionnaire [NAVQ]). NAVQ requires patients to indicate their level of difficulty in performing common near-vision and intermediate-vision tasks without the use of reading spectacles (0= no difficulty; 3= extreme difficulty) and to rate overall satisfaction with near vision (0= completely satisfied; 4= completely unsatisfied). The summated score is adjusted to a Rasch score (0 to 100 logits) using a conversion table. 0 indicates no difficulty with any of the near tasks and 100 indicates extreme difficulty with all near activities.
Results:
Multifocal IOL was implanted in 20 patients and trifocal IOL was implanted in another 20 patients in the study group. The mean monocular UDVA in the whole study group was 1.05 and the mean binocular UDVA was 1.26. The mean monocular UNVA was 0.99, while the mean binocular UNVA was 1.0. The mean NAVQ Rasch score for satisfaction with near vision activities was 20.06 ± 11.7 logits ( range 0 to 41.22 logits). The mean overall satisfaction score with near vision was 0.7 in the whole study group. Significant difference (p = 0.01) was found between mean Rasch score of patients with multifocal IOL (22.08) and patients with trifocal IOL (17.96). The mean overall satisfaction score with near vision was significantly better in the group of patients with trifocal IOL (0.4) compared to group of patiens with multifocal IOL (0.85).
Conclusions:
Subjective satisfaction with near vision functions after bilateral RLE was high in the present study. However, trifocal IOLs showed a higher level of patients´ satisfaction with near vision than multifocal IOLs. Better results of trifocal IOLs may be due to improved intermediate visual ability.
