First outcomes of ametropic presbyopia correction with implantation of trifocal toric lens

Purpose:

The aim of our study was to evaluate first outcomes of trifocal toric intraocular lens AT LISA tri toric 939MP (Zeiss) implantation for a correction of ametropic presbyopia in patients after refractive lens exchange (RLE). We have studied functional outcomes and efficacy of postoperative vision, rotational stability of the lens and postoperative complications.

Setting:

European Eye Clinic Lexum, Prague, Czech Republic

Methods:

The study enrolled first 5 patients (8 eyes) who had implantation of a new type of lens AT LISA tri toric 939MP (plate-haptic design). The mean patient age at the time of operation was 58 years (range: 41 to 64 years). All patients had uneventful extraction of clear lens by phacoemulsification and capsulorhexis with a diameter of 5mm. Dioptric power of the lens was calculated with online ZEISS Z-CALC calculator based on parameters from IOL Master device. Three patients had bilateral implantation while one had unilateral implantation of trifocal toric IOL. Due to an unequal degree of astigmatism in one patient, one of his eyes with higher astigmatism had implantion of a toric version of the trifocal AT LISA, while the other eye received a non-toric version (lower degree of astigmatism was resolved with limbal relaxing incisions - LRI). Postoperatively, uncorrected (UDVA) and best corrected (CDVA) distance visual acuity, uncorrected near (UNVA) and intermediate (UIVA) visual acuity and astigmatism were evaluated.

Results:

The mean decimal monocular UDVA increased from preoperative 0.41 ±0.30 to 0.79 ±0.25 one week after the treatment. The mean postoperative CDVA was 0.98 ±0.12. The mean UNVA (Jaeger) corresponded to J1-J2. The mean UIVA was J3. Preoperative astigmatism obtained from IOL Master was -1.77 ±0.42 D. Manifest astigmatism decreased from preoperative -1.13 ±0.53 D to postoperative -0.31 ±0.41 D. Postoperatively, implanted lenses were on the right position in 7 eyes. In one eye, reposition of the lens was required 1 week after the surgery due to its rotation of 15 degrees from intended position. No cases of IOL decentration were recorded, no other complications occurred.

Conclusions:

Our study showed that correction of presbyopia in eyes with high level of astigmatism with implantation of trifocal toric lens AT LISA tri toric 939MP is a suitable and effective solution. This method provides very good quality of vision at all three distances. FINANCIAL INTEREST: NONE
