Subluxated traumatic cataract: intracapsular extraction and scleral fixated posterior chamber intraocular lens implantation

Abstract Details
Purpose:
To report the surgical approach of an intracapsular cataract extraction and scleral fixated posterior chamber intraocular lens implantation (SF IOL), in a 62-years old female patient, with a subluxated traumatic cataract.
Setting:
Centro Hospitalar e Universitário de Coimbra, Ophthalmology Department
Methods:
The surgery was performed under general anesthesia. Initially, a 23-gauge trocar cannula was inserted, at 7 o'clock, 4mm from the limbus. Then, a superior limbal peritomy and a scleral tunnel incision were performed, the latter at 2mm from the limbus and 8mm width. Through this incision the subluxated cataract was extracted, with a lens loop. The incision was sutured, the remaining trocars were inserted at 2 and 10 o'clock and a 23-gauge pars plana vitrectomy (PPV) was performed.
After the PPV, two scleral grooves were created, horizontally at 3 and 9 o'clock. The SF IOL was inserted into the anterior chamber through the superior scleral tunnel previously created, centered in the posterior chamber and sutured to the sclera.
Results:
At one-year follow-up, best-corrected visual acuity was 20/30, without complications.
Conclusions:
Scleral fixated posterior chamber intraocular lens implantation is clinically safe, effective and a viable alternative to correct aphakia in patients without capsular support. FINANCIAL INTEREST: NONE
