Purpose:

The calcification of hydrophilic IOLs is a very rare complication. We report about the findings in opacified Euromaxx ALI313Y IOLs (Argonoptics) using light and scanning electron microscopy as well as x-ray spectroscopy.

Setting:

David J Apple International Laboratory for Ocular Pathology, University Eye Clinic Heidelberg, Germany.

Methods:

Calcified Euromaxx ALI313Y IOLs (Argonoptics) had been explanted because patients suffered from a decrease in visual acuity. These IOLs were sent to our laboratory and examined using light and scanning electron microscopy. The composition of the deposits was analyzed by means of x-ray spectroscopy.

Results:

Macroscopically, the entire optic was opacified. Light and scanning electron microscopy revealed numerous fine, granular, crystalline-like deposits which were always distributed in a line parallel to the anterior and posterior surface of the IOLs. X-ray spectroscopy could prove the deposits to consist of calcium and phosphate.

Conclusions:

The calcification of hydrophilic IOLs only occurs rarely. The exchange of the IOL is the only therapeutic option in symptomatic patients. The exact chemical composition of the deposits can be assessed by means of x-ray spectroscopy. FINANCIAL INTEREST: NONE
