Comparative analysis after bifocal or trifocal diffractive intraocular lens

Purpose:

To compare the visual performance after phacoemulsification with implantation of either a bifocal (AT LISA 801) or trifocal lens (AT LISAtri 839MP) in a follow-up period of 3 months

Setting:

Premium Clinic Teplice / Department of Ophthalmology, Havlickuv Brod, Czech Republic

Methods:

30 eyes of 15 patients were enrolled in each study group. All patients underwent uncomplicated phacoemulsification surgery with bilateral implantation of either the AT LISA 801 (bifocal group) or AT LISAtri 839MP (trifocal group) intraocular lens (IOL). The study protocol included preoperative and postoperative measurement of uncorrected and corrected distance (UDVA/CDVA, 4 m), intermediate (UIVA/CIVA/DCIVA, 66 and 80 cm) and near visual acuity (UNVA/CNVA/DCNVA, 33 and 40cm) and ocular aberrations, as well as postoperative measurement of photopic contrast sensitivity (CS). Defocus curves were also evaluated.

Results:

No statistically significant differeneces were found betweeen the groups regarding UDVA (p=0.21) and CDVA (p=0.37). UNVA, CNVA and DCNVA measured at 33 and 40 cm were significantly better in the trifocal group compared to bifocal group (p<0.01). UIVA, CIVA and DCIVA measured at 66 cm were also significantly better in the trifocal group (pŁ 0.03). Likewise, significantly better values of UIVA and DCIVA measured at 80 cm were found in the trifocal group (p<0.01). In contrast, no statistically significant differences between groups were found in CIVA (p=0.21). No statistically significant differences between groups were found either in postoperative sphere (p=0.85) and spherical equivalent (p=0.22). Regarding the defocus curve analysis, visual acuity was significantly better in the trifocal group compared to the bifocal for defocus levels of - 0.5D (p=0.01), -1.0D (p<0.01), and - 1.5D (p=0.04).

Conclusions:

Bifocal and trifocal AT LISA IOLs are able to provide a complete near and distance visual rehabilitatcion, but the trifocal lens provides better intermediate vision compared to the bifocal . FINANCIAL INTEREST: NONE
