Intra-patient comparison of two hydrophobic acrylic intraocular lenses

Purpose:

This study was designed for long term intra-patient comparison of two hydrophobic acrylic intraocular lenses (IOL) regarding posterior capsule opacification (PCO) and contrast sensitivity

Setting:

Department of Ophthalmology Istanbul Faculty of Medicine Istanbul University

Methods:

Thirty eyes of 15 patients undergoing phacoemulsification with IOL implantation were included in this prospective, comparative study. At the end of surgery, one eye of each patient received an AcrySof IOL and the other eye a Sensar AR40 IOL. Mean follow-up was 14.5 months (1 year-3 years). Patients were examined for visual acuity, keratometry, contrast sensitivity. IOL was examined at slit lamp for centration and PCO by the same examiner uninformed about IOL. Contrast sensitivity was assessed under mesopic conditions using a chart. PCO was graded using a system ranging from none/slight to severe. Chi-square and Mann Whitney U tests were used for comparisons.

Results:

Mean age was 65.17+20 years. There were no differences in visual acuities obtained with the two IOLs preoperatively and at 1 month and 1 year postoperatively. Mean surgically induced astigmatism was similar. The differences between the 2 groups regarding contrast sensitivity were not significant. PCO was present in 3 eyes (16%) in the AcrySof group and in 8 eyes (54%) in the Sensar group. The difference between the 2 groups was highly significant (p<0.0001). The PCO in the AcrySof group was mild (Grade 2) and PCO in the Sensar group was mild (Grade 2) in 7 eyes and moderate in 1 eye (Grade 3). No eye underwent YAG laser capsulotomy.

Conclusions:

When surgery and patient related factors are kept constant, geometric design of IOL seems to be the most important factor in reducing PCO. Square edge and posterior convex optic are superior to other designs in preventing PCO. FINANCIAL INTEREST: NONE
