Did we cure amblyopia in an adult?

Abstract Details
Purpose:
We present the case of 22-year-old male patient, who underwent refractive surgery - implantation of toric Visian implantable collamer lens (ICL). ICL was implanted into the eye with presumed amblyopia. After the implantation there was unexpected improvement in best corrected visual acuity (BCVA).
Treatment options for amblypia in adults are limited. There are a few reports from literature of improvement in BCVA in amblyopic patients after excimer laser surgery.

Preoperative examination:
Visual acuity of right eye: 0,5, +0,50 Dsph, -3,25 Dcyl x 10 degrees 1,0.
Visual acuity of left eye: counting fingers 0,5 m, -12,50 Dsph, -4,0 Dcyl x 170 degrees 0,5. Autorefraction in cycloplegia - right: +1,0 Dsph, -3,50 Dcyl x 12 degrees, left: -12,25 Dsph, -4,50 Dcyl x 171 degrees.
Intraocular pressure 20 / 18 mmHg, optical media were clear, right eye fundus physiological, left myopic, pachymetry 542 / 548 μm. There was significant anisometropia (right mixed astigmatism, left myopic astigmatism) and left eye amblyopia. Previous medical records from children university hospital confirmed, that patient was never able to see more than 0,5 line from optotype with his left eye.
We performed following surgical procedures: right eye - laser epithelial keratomileusis (LASEK) with mitomycine C, left eye - ICL implantation.
On the first day after ICL implantation uncorrected visual acuity (UCVA) of left eye was 1,0. There is no significant change in UCVA of left eye after 3 months of follow-up.
Conclusions:
Unexpected postoperative improvement of visual acuity after ICL implantation above preoperative BCVA questions amblyopia diagnosis. Despite this fact patient was never able to see with his presumably amblyopic eye with any correction more than 0,5 line of optotype and only after ICL implantation he was able to achieve 100% central visual acuity. Adult patients with anisometropic amblyopia could benefit from the ICL implantation with significant improvement of the visual acuity. FINANCIAL INTEREST: NONE
