Evaluation of our first 100 implanted Visian ICL V4c lenses (implantable collamer lens, STAAR Surgical) in patients with myopia and astigmatic myopia concerning predictability, safety and visual acuity

Purpose:

Phacic IOLs (Intaocular Lenses) are becoming more and more popular and meanwhile the Implantation of the Visian ICL is the fastest growing refractive procedure. In the past the ICL had some disadvantages e.g. the necessity of an iridectomy, the risc of cataract formation and the risc of high intraocular pressure. This was the impetus for Staar Surgical to produce the new ICL V4c to get over these disadvantages. After having implanted more than 100 ICL V4c the aim of this study was to evaluate the Safety, Predictability, the Visual Acuity and long term stability of this new lens ICL V4c.

Setting:

Eye- Surgery- Centre Lohr am Main, Germany

Methods:

This retrospective review included patients who underwent bilateral ICL- V4c Implantation. Patients suffered from Myopia and Astigmatism, had no other ocular pathology and were not suitable for a Laser Vision Correction. Patients were between 18 and 50 years old. Subjective refraction was measured preoperatively, 1, 3 and 12 months after surgery. White- to- White measurement was performed with the IOL-Master (Carl- Zeiss- Meditec)

Results:

Mean preoperative spherical equivalent (SE) refraction was -6.81 ± 1.62 D (range from -1.5 D to -11.5 D). Preoperative Astigmatism was -2.3 ± 0.72 D (range from 0 to -5,5 D). Postoperatively, there was a statistically significant decrease in the SE value, reaching respectively 0.5 ± 0.49 D (range: -1.0 to +1.25 D) and 0.4 ± 0.41 D (range: -0.875 to 1.25 D) after 1 month and 3 months. The cylinder refraction was measured -0.56 ± 0.44 D after 1 month and -0.45 ± 0.44 D after 3 months. We had 4 eyes that underwent a so called touch-up with a LASIK to correct the residual refraction. There was so far no cataract formation in any eye. We had ten eyes with a mild intraocular high pressure on the first day after surgery (23 to 26 mmHg), that went down within 24 hours. No iridectomy was necessary.

Conclusions:

The implantation of the Visian ICL V4c resulted in excellent and stable refractive outcomes. Patients were highly satisfied with their postoperative vision and the lens provided a very good uncorrected visual acuity. Until now we have no eye with a lens opacity after operation and no severe intraocular high pressure without the necessity of performing an iridectomy.
